• Contractors must first attend EHS induction prior to applying for EHS Induction card. Please furnish Form 3 that has been acknowledged by both EHS and contractor during EHS Induction.
• For renewal of EHS cards, contractor to attach the most recent EHS cards as evidence that they have attended EHS Induction and they have worked in Pavilion in the past one year.
• Contractor must re-attend EHS Induction if EHS cards have expired >1 year to be updated on current House Rules & Regulations.
